Who lends to chiropractors, physical therapy clinics and med spas?
MedStream Capital does. We underwrite and fund working capital ($25,000 to $2,000,000) and lines of credit ($25,000 to $500,000) in-house for chiropractic and integrated-care practices, physical, occupational and speech therapy clinics, and med spa and aesthetics practices. The floor is 6 months in business and $15,000 a month in deposits, with no minimum credit score; term loans, SBA 7(a) and claims factoring are arranged through bank and specialist partners.

Three practices, three different cash cycles
A lender that treats “healthcare” as one business will misread all three of these. We underwrite each on the way its money actually moves.
Chiropractic and integrated care. Mostly cash-pay and care-plan revenue, with a commercial insurance tail. Personal-injury and attorney-lien files are the long pole — they can take six to eighteen months to collect, and they are the most common reason a chiropractic practice with good production has no cash. A working capital advance bridges the lien book without anyone touching the case files.
Physical, occupational and speech therapy. Claim-driven and structurally squeezed. The 2026 Medicare conversion factor for most outpatient therapy clinics is $33.4009, still below the $36.09 of 2020. High visit volume on thin per-visit economics means a 30-day swing in collections is the whole margin, so a receivable bridge or a standing line is the usual answer.
Med spa and aesthetics. 100% cash-pay: no payer, no claim, no receivable. The pressure is inventory and demand, and it peaks from October through December. Gross margins run 50% to 65%. A line of credit drawn for the fourth-quarter injectable buy and repaid by January costs only the weeks it is out.

By practice type
What each practice borrows for, with published files.
| Practice type | Cash cycle | Typical uses and amounts | Typical published file |
|---|---|---|---|
| Chiropractic & integrated care | Cash-pay and care plans; lien files take 6 – 18 months | Lien receivable bridge $40,000 – $200,000; adding a nurse practitioner or medical service line $75,000 – $250,000; second location $150,000 – $500,000 | 13 years in business, $70,000/mo deposits, $85,000 advance over 12 months |
| Physical, occupational & speech therapy | Claim-driven; thin per-visit margins | Receivable bridge $40,000 – $150,000; treatment room or second clinic $100,000 – $400,000; hiring a therapist ahead of caseload $50,000 – $120,000; billing-company transition $25,000 – $80,000 | 17 years in business, $85,000/mo deposits, $112,000 advance over 12 months |
| Med spa & aesthetics | 100% cash-pay; peaks October – December | Injectable inventory before Q4 $25,000 – $100,000; second treatment suite $100,000 – $350,000; marketing ahead of a launch $25,000 – $75,000; second location $250,000 – $700,000 | 7 years in business, $165,000/mo deposits, $150,000 advance over 12 months |
What every one of them needs to qualify
The criteria are the same published floor for all three: 6 months in business, $15,000 a month in deposits across all business accounts, a U.S. business checking account in the practice’s legal name, and an active state professional or facility license for the practice or its supervising provider. That last line matters for med spas, where the license may sit with a supervising provider rather than the owner. There is no FICO minimum, and the soft pull at application does not touch your personal score.
Practices under 24 months old are funded, typically at $25,000 to $75,000. A line of credit needs $20,000 a month in deposits rather than $15,000.
In-house versus partner, stated plainly
Working capital and lines of credit are underwritten and funded by MedStream Capital. Term loans and SBA 7(a) are arranged through partner banks; claims factoring through a specialist partner under a Business Associate Agreement. Those carry their partners’ criteria — for example 24+ months and a 600+ FICO on a term loan — and we identify the funding party before you sign.
For a second clinic or a buy-in, a partner term loan will often beat an advance on cost if you have the history and credit for it. We read the file first and say so.

FAQ
Questions that usually follow.
Does MedStream Capital fund med spas?
Yes. MedStream Capital underwrites and funds working capital and lines of credit in-house for med spa and aesthetics practices. You need 6 months in business, $15,000 a month in deposits and an active state license for the practice or its supervising provider. There is no minimum credit score.
Can a chiropractor get funding against personal-injury cases?
Not against the case files themselves — we never review them. MedStream Capital funds a working capital advance on your business bank deposits, and practices use it to carry a lien-heavy book while cases settle. A personal-injury and lien receivable bridge typically runs $40,000 to $200,000.
Can a physical therapy clinic get funding while waiting on Medicare?
Yes. A receivable bridge for a therapy clinic typically runs $40,000 to $150,000, underwritten on four months of bank statements rather than on claims. Decisions come the same business day on complete files received before 2:00 p.m. ET, and funds wire 24 hours after a signed contract.
What credit score does a chiropractor, PT clinic or med spa need?
None for MedStream Capital working capital or a line of credit. We underwrite deposits. Partner term loans need a 600+ personal FICO and SBA 7(a) needs 680+.
Who actually funds my file?
MedStream Capital underwrites and funds working capital and lines of credit itself. Term loans and SBA 7(a) are funded by partner banks, and claims factoring by a specialist partner under a Business Associate Agreement. We tell you which before you sign anything.
